When Should You Perform CPR?

CPR is a life-saving practice used when a person’s heart stops beating or they are not breathing normally. Knowing when to start CPR is critical because every second without circulation can affect the brain, heart, and other vital organs.

When a Person Is Unresponsive

One of the first signs that CPR may be needed is unresponsiveness. If someone collapses or does not respond when you tap them and shout, they may be experiencing a medical emergency.

Before starting CPR, check the scene for safety. Then try to wake the person by speaking loudly and tapping their shoulder. If they do not respond, call for help immediately or direct someone nearby to call 911 and get an AED.

When Breathing Is Absent or Abnormal

CPR should be performed when a person is not breathing or is only gasping. Gasping, also called agonal breathing, is not normal breathing and can happen during cardiac arrest.

Signs that CPR may be needed include:

  • No breathing
  • Only occasional gasps
  • Unresponsiveness
  • No signs of normal movement
  • Sudden collapse

If you are unsure whether the person is breathing normally, it is better to begin CPR than to wait too long.

When There Is No Pulse for Trained Providers

Healthcare providers and trained rescuers may check for a pulse before beginning CPR. If there is no pulse, CPR should begin right away. For lay rescuers, checking responsiveness and breathing is usually the priority because pulse checks can be difficult without training.

When Waiting for Emergency Help

CPR helps the blood flow to the brain and vital organs until emergency personnel arrive. If an AED is available, turn it on and follow the prompts. Continue CPR until the person starts normal breathing, emergency responders take over, an AED instructs you to pause, or you are physically unable to continue.

Acting Quickly Can Save a Life

The decision to perform CPR can feel overwhelming, but immediate action matters. If someone is unresponsive and breathing abnormally, call 911, start CPR, and use an AED if available.
